UK accident report finds inspection plan was not ‘mutually understood’

A Filipino crew member died while inspecting a lift on a Carnival-managed cruise ship off the Canary Islands, an accident report has revealed.

The electrical technician was killed on the 186,000-gt Arvia (built 2022) on 26 October last year, according to the UK Marine Accident Investigation Branch (MAIB).

The Bermuda-flagged passenger vessel, operating under Carnival’s P&O Cruises brand, was heading from Southampton to Santa Cruz de Tenerife.

MAIB said the lift was being checked after overnight repairs.

It had been returned to normal operation for testing.

The lift moved in response to a stored call signal while the crew member was inside the shaft.

The investigation found that the inspection was not supported by a clearly communicated and mutually understood plan.

MAIB said: “Positive control of the lift had not been established before the crew member entered the lift shaft, and the work relied on self-resetting door interlocks that did not provide an effective barrier against lift movement.”

Carnival UK has since taken action to reinforce lift maintenance procedures, communication arrangements and training requirements across its fleet.

A recommendation has been made to the company to review the use of self-resetting lift door interlocks as a safety barrier.

The electrical technician, Lixter Dave Visaya Mahinay, was 35 years old and serving his second contract with Carnival UK.
