Seafarer was fatally injured after falling 12 metres while cleaning the crane cabin

An Indian investigation has highlighted a series of safety failings after a seafarer died after falling through the glass floor of a crane cabin on a bulk carrier during routine cleaning.

The seafarer removed a protective grate on the floor of the cabin but the exposed glass panel was not strong enough to hold his weight, according to an investigation by the Indian directorate general of shipping.
